All personal details provided in this Application Form 1A — including my name, email address, phone number, residential address, gender, and government-issued photo ID — are true, accurate, complete, and current to the best of my knowledge. I understand that submitting false or misleading information may result in the rejection of my application or cancellation of any enrolment.

I consent to the college collecting and retaining my personal information and identity document solely for the purpose of internal record keeping, student identification, and enrolment verification.
